Overview:
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) and Registered Nurse (RN) to join our healthcare team. Duties:
- Administer medications and treatments as prescribed by physicians
- Monitor patient vital signs and report any significant changes
- Assist with patient assessments and provide appropriate nursing interventions
- Perform basic life support measures in emergency situations
- Access and maintain intravenous (IV) lines as needed, RN only
- Provide patient/family education on various health topics
- Practices nursing with respect for individual, cultural, and spiritual differences

- Maintain accurate and up-to-date medical documentation

- Communicates effectively and in accordance with agency requirements

- Supervises applicable home care team members as delegated by RN Case Manager

Requirements:
- Valid LPN or RN license in Minnesota
- Proficiency in medical terminology and documentation
- Strong knowledge of anatomy and physiology
- Ability to assess patient needs and develop individualized care plans
- Excellent communication and interpersonal skills
- Basic life support (BLS) certification
- Ability to work independently, self-motivated, resourceful, problem solver and prioritization skills are helpful
