Key Responsibilities:
- Champion of Care: Coordinate and facilitate the transition of patients from hospital to home health care, ensuring a seamless and stress-free experience.
- Relationship Builder: Develop and nurture strong relationships with hospital staff, including discharge planners, case managers, and other healthcare professionals.
- Patient Advocate: Assess patients' needs and ensure all necessary arrangements are made for a smooth transfer to home health services.
- Educator Extraordinaire: Educate patients and their families about home health services and the transition process, empowering them with the knowledge they need.
- Liaison Leader: Serve as a liaison between the hospital, patients, and home health care teams to ensure continuity of care.
- Sales Superstar: Conduct sales calls and presentations to promote our home health services to hospitals and healthcare providers.
- Team Collaborator: Work closely with the home health care team to develop and implement individualized care plans for patients.
- Detail-Oriented Professional: Maintain accurate and up-to-date patient records and documentation.

Qualifications
- Current LPN or RN license in the state of Oklahoma.
- Proven experience in a healthcare setting, preferably in a hospital or home health environment.
- Strong interpersonal and communication skills.
- Sales-oriented mindset with the ability to build and maintain relationships with healthcare professionals.
- Ability to work independently and as part of a team.
- Excellent organizational and time management skills.
- Valid driver's license and reliable transportation.
- Demonstrated familiarity with home healthcare services, referral sources and payors.
- Ability to navigate through EMR.
